SUPPORTED LIVING (SL) PROGRAM
Provides assistance and training in the following areas:
- Budgeting/finances
- Menu planning
- Shopping
- Cooking
- Cleaning
- Laundry
- Personal self help
- Emergency management
- Social skills
- Conflict resolution
- Leisure time planning & management
- Medical appointments, follow-up and medication guidance
- Public transportation training and scheduling.
Supported Living Coaches follow up with clients on a regular basis, usually a minimum of five times per month. Referrals are accepted from the Agency for Persons with Disabilities/Medicaid Waiver program and from private sources.
