SUPPORTED LIVING (SL) PROGRAM

Provides assistance and training in the following areas:

  • Budgeting/finances
  • Menu planning
  • Shopping
  • Cooking
  • Cleaning
  • Laundry
  • Personal self help
  • Emergency management
  • Social skills
  • Conflict resolution
  • Leisure time planning & management
  • Medical appointments, follow-up and medication guidance
  • Public transportation training and scheduling.

Supported Living Coaches follow up with clients on a regular basis, usually a minimum of five times per month. Referrals are accepted from the Agency for Persons with Disabilities/Medicaid Waiver program and from private sources.
